Cost is $10.00 for certificate the cost is the same for SMAR members and non-members. Payments must be in the form of cash and handed in at the time of sign in.

CONTINUING EDUCATION RULES:

1. Remember we will have breaks, so please turn cell phones/pagers OFF(not vibrate). Aside from breaks allowed by instructor, you will NOT be permitted to leave the room during class unless it is an emergency.
2. Please fill out your monitor card. In the event you need to leave the classroom, you must give your card to the monitor. Absences longer than 5 minutes will result in loss of CE hours.
3. Leave the monitor card at your seat/desk when the instructor takes breaks. If you do not return at the time specified by the instructor, it will be noted.
4. Turn in the monitor card at the end of class in order to recieve your Continuing Education Certificate. Please note: Failure to turn in a monitor card at the end of class will result in the loss of Continuing Education Credit.

PLEASE REMEMBER - YOUR MONITOR DOES NOT HAVE THE AUTHORITY TO FORGIVE YOUR ABSENCE FROM CLASS. PLEASE DO NOT ASK HIM/HER.
